    • Experimental replacement of thoracic esophageal segment with a biomaterial artificial esophagus in dogs

      Abstract:<正>Objective:To implant 80 mm-long artificial esophagi constructed of biomateriai in dogs,observe the perioperative survival rates and the incidence of postoperative complications,and study the mechanisms of postoperative healing.Methods:Specimens of the implanted esophagus,the"neo-esophagi",were taken for histopathologic study 1,3,6,12,24 months after operation.Results:The incidence of anastomotic leakage after the artificial esophagus implantation was 3.33%.The perioperative survival rate was 96.67%.The incidence of postoperative stenosis in the"neo-esophagi"was 81.48%;the stenoses were treated by expanding with esophagoscopy and implanting a stent.Epithelization of the mucosa in the"neo-esophagi"was completed in 3 to 6 months after surgery Structures such as submucosal muscle layers,mucous glands,nerve fibers,capillaries,etc.were regenerated after 12 months,and then reconstruction of the fibrous connective tissue layer was completed.Conclusion:Implanting a biomaterial artificial esophagus accomplishes safe reconstruction of defects in the esophagus.Advanced cellular structure of"neo-esophagus"can be regenerated after 1 year.Postoperative stenosis,which is related to hyperplasia and retraction of scar tissue,is still the most common complications which limiting the clinical application of the artificial esophagus.

      Abstract:<正>Objective:To determine the changes of gastrointestinal migrating motor complex(MMC)during the early stage of cholesterol gallstone formation.Methods:Totally 45 healthy adult guinea pigs were randomly and equally divided into 3 groups,that is,normal control,group B(1% cholesterol diet for 2 weeks)and group C(1% cholesterol diet for 4 weeks).Three pairs of silver electrodes were inserted into the gastric antrum,duodenum and jejunum of every animal respectively.Record of gastrointestinal MMC of these guinea pigs were recorded and analyzed. Gallbladder bile was taken to detect the formation of cholesterol gallstone.Results:There were only 29 guinea pigs living.The MMC cycle time and the duration of phaseⅡwere prolonged significantly and gradually compared with the control group(P<0.01),but the duration of phaseⅢbecame significantly and gradually shorter.Conclusion: During the early stage of cholesterol gallstone formation,the MMC cycle time and the duration of phaseⅡare prolonged and the duration of phaseⅢis shortened

      Abstract:<正>Objective:To test the characteristics of hyaluronic acid(HA)derivative cross-linked by polyethylene glycol films of low water content.Methods:The cross-linked HA film with 200μm thickness was got at atmospheric pressure at 25℃for 5 d.After dried,cross-linked films of 10 mm×10 mm were weighed and immersed in phosphate buffered saline (PBS pH 7.45)at 37℃for 24 h.Then the solution fraction and water content were estimated.Meanwhile,cross-linked HA derivative films were immersed in phosphate buffered saline(PBS:pH 7.45)at 37℃for determined time and then implanted subcutaneously in the back of white rats to test in vitro or in vivo degradation characteristic.Results and Conclusion:HA hydrogel cross-linked by polyethylene glycol with water content is as low as 60% and this kind of HA derivative has a slow degradation rate.

    • Therapeutic effectiveness of pediatric renal transplantation in 63 cases

      Abstract:<正>Objective:To explore the characteristic of operation,intra-operation treatment and the application of immunosuppressant in pediatric renal transplantation in order to improve therapeutic effectiveness.Methods:From March 1986 to October 2006,the clinical data of 63 children who underwent renal transplantation in our hospital were retrospectively analyzed.Results:The 1-,3-,5-,10-year graft survival rates were 98.4%,90.5%,88.9% and 68.3%, respectively.And the corresponding patient survival rates were 100%,95.2%,92.1%,71.4%.The body weight increased 4 to 12 kg and the body height grew up 2 to 6 cm during the first year post-transplantation.The main complications in the first year post-transplantation were hypertension(26/63,41.3%),crinosity(14/63,22.2%), drug-induced hepatic injury(11/63,17.5%),gingival hyperplasia(10/63,15.8%),pulmonary infection(9/63,14.3%), bone marrow suppression(5/63,7.9%),herpes(4/63,6.3%)and diabetes(3/63,4.8%).Conclusion:Renal transplantation is a preferred method for the treatment of children in end-stage renal disease(ESRD).Good tissue matching,proper operative time and pattern,peri-operactive care were essential to success,as well as appropriate immuno-suppressant strategy and good compliance.

      Abstract:<正>Objectives:To explore possible mechanisms of connexin40(Cx40)remodeling by detecting Cx40 mRNA expression of the crista terminalis and left atrium(LA)in patients with rheumatic heart disease(RHD)associated chronic atrial fibrillation(AF).Methods:Twenty patients were enrolled in this study,who underwent surgical operation for RHD-associated mitral disease,including 10 with sinus rhythms(rhythm group)and 10 with AF(AF group).Another 6 patients with non-RHD sinus rhythms were divided into the control group.A small amount of myocardial tissue was cut from the crista terminalis and the LA posterior wall during the valvular replacement operation.Cx40 mRNA expression was assayed by real-time fluorescent quantitation polymerase chain reaction (RT-PCR).Results:There was no significant difference in Cx40 mRNA expression in the crista terminalis and LA posterior wall between the 3 groups,and there was no significant difference in Cx40 mRNA expression between the crista terminalis and LA within each group.Conclusion:Based on the finding in previous studies that there existed evident remodeling of atrial Cx40 protein in patients with chronic RHD,the results of the present study suggest that the mechanism of Cx40 remodeling probably lies in the post transcriptional level.

    • Atorvastatin attenuates oxidative stress in Alzheimer’s disease

      Abstract:<正>Objective:To investigate serum level of SOD,MDA,ox-LDL,AchE and Ach in AD,to study atorvastatin influence on serum level of SOD,MDA,ox-LDL,AchE and Ach in AD and its neuroprotection mechanisms.Methods Subjects were divided into:normal blood lipid level group with Alzheimer’s disease(A),higher blood lipid level group with Alzheimer’s disease(AH),normal blood lipid level Alzheimer’s disease group with atorvastatin treeatment(AT), higher blood lipid level Alzheimer’s disease group with atorvastatin treeatment(AHT).Ox-LDL was measured by enzyme linked immunosorbent assay;SOD,MDA,ox-LDL,AchE,Ach and blood lipid level in AD was measured by biochemistry.Results:The serum level of MDA,AchE in AH group after atorvastatin treatment is lower;The serum level of SOD,Ach in AH group is more increased than that of in A group;The serum level of ox-LDL in AH,A groups is lower than that of in A group;The dementia degree is lower after atorvastatin treatment.Conclusion:Atorvastatin can decrease serum level of MDA,AchE and ox-LDL,and increase that of SOD,Ach,and attenuate dementia symptom in AD,especially,with hyperlipemia.The hypothesis of atorvastatin neuroprotection is concluded that atorvastatin may restrain free radical reaction and retard oxidation in AD.

      Abstract:<正>Objective:To explore the roles of persistent systemic inflammation in atherosclerosis and the effects of saponins of Panax Notoginseng(PNS)on this process in rabbits.Methods:Thirty rabbits were divided randomly and equally into 6 groups,i.e.,control,high-fat diet,inflammation,aspirin,PNS and simple-inflammation group.All the animals except that in control group and simple-inflammation group were fed with high-fat diet for 8 weeks.Based on that, rabbits in inflammation,aspirin and PNS groups were treated with zymosan injection(10 mg/kg,i.p.).Normal saline was given to rabbits in control group.Besides zymosan injection,animals in aspirin and PNS group were administrated with aspirin(12 mg/kg,i.g.)and PNS(120 mg/kg,i.g.)respectively.The animals in simple-inflammation group were treated with zymosan injection(10mg/kg,i.p.)and fed with normal diet.The atherosclerosis lesion in aortas was observed by SudanⅣstaining.Serum total cholesterol,triglyceride(TG),TNF-αand activity of post-heparin lipoprotein lipase(LPL)were measured at the end of the 4th and 8th week after an overnight fast.Results:Compared with high-fat diet group,the area of atherosclerosis lesion,serum TG and TNF-αwere markedly increased in rabbits of inflammation group,and the activity of LPL was decreased remarkably.Serum TNF-αlevel was negatively correlated with the activity of post-heparin LPL(r=-0.708,P<0.01).The area of atherosclerosis,serum TG and TNF-αwere decreased in aspirin and PNS group compared with that in inflammation group,and the activity of LPL was increased remarkably.Compared with control group,serum TG and TNF-αwere markedly increased in simple-inflammation group,while LPL activity was decreased.Atherosclerotic lesion did not occur in simple-inflammation group. Conclusion:Persistent systemic inflammation could accelerate the formation of atherosclerosis lesion in aortas,which partly depend on the decreasing of the activity of post-heparin lipoprotein lipase.PNS could improve the changes caused by inflammation.

    • Effect of berberine on Cdk9 and cyclin T1 expressions in myocardium of diabetic rats

      Abstract:<正>Objective:To investigate the effect of berberine,one of the main alkaloids of Rhizoma coptidis,on myocardial morphology and the expressions of cyclin-dependent kinase 9(Cdkg)and cyclin T1 protein in the myocardium of type 2 diabetic rats.Methods:Type 2 diabetes mellitus rats were induced by an injection of 35 mg/kg streptozotocin(STZ) and a high-carbohydrate/high-fat diet for 16 weeks.Diabetic rats were given low-,middle-,high-dose berberine(75, 150,300 mg/kg),fenofibrate(100 mg/kg)and rosiglitazone(4 mg/kg)for another 16 weeks,respectively.The myocardium structure was observed with hematoxylin & eosin(H&E)staining and Cdk9 and cyclin T1 protein expressions were detected by immunohistochemistry.Results:Middle-dose,high-dose berberine improved myocardial hypertrophy and interstitial fibrosis of diabetic rats.Cdk9 and cyclin T1 protein were significantly lower in diabetic myocardium than in control one(P<0.01),and middle-dose,high-dose berberine and fenofibrate obviously increased both Cdk9 and cyclin T1 expression to near control level(P<0.01).Conclusion:Berberine modulates Cdk9 and cyclin T1 protein expression in diabetic myocardium which may contribute to ameliorate myocardium damage.

      Abstract:<正>Objective,To report the occurrence,management and outcome of late-onset traumatic dehiscence and dislocation of laser assisted in situ keratomileusis(LASIK)flaps.Treatment and Results:One patient occurred late-onset LASIK corneal flap dislocation after ocular trauma 7days after surgery.The flap was lifted,stretched,and repositioned after irrigation and scraping of the stromal bed and the underside of the flap.A bandage contact lens was placed,and topical antibiotic and corticosteroids were given postoperatively.The dislocated corneal flap was successfully repositioned in the case.The dislocated flap was repositioned 7 days after the trauma,and the patient recovered his uncorrected visual acuity(UCVA)of 10/20,20/20 day 1 and day 20 after the procedure,of 20/20 20 days later and had a well-positioned flap with a clear interface.Diffuse lamellar keratitis developed in the patients that resolved with the use of topical corticosteroids.Conclusion:Laser in situ keratomileusis corneal flaps are vulnerable to traumatic dehiscence and dislocation,which should be pay more attention to it for us.

      Abstract:<正>We report the case of a ten-year-old boy who had often presented with respiratory distress since born.Chest X-ray showed an enlarged right ventricle.Echocardiography demonstrated characteristic features of Failot’s tetralogy and the left pulmonary artery could not be visualized.However,cardiac catheterization disclosed that the left pulmonary artery had an anomalous origin in the ascending aorta.The patient underwent total surgical correction of the defects.Nine days later,he was discharged.We present the results of a literature review of the incidence,physiopathological,clinical, diagnostic,and surgical characteristics of this rare disease.
