      Abstract:The German wound healing society ICW(Initiative Chronische Wunden) started a training program for nurses in 2005.Certified by TüV Rheinland,the courses are regularly audited and the quality is ensured.More than 30,000 nurses attended these courses in Germany.In three Chinese hospitals the ICW courses have been adopted.A close collaboration between Chinese and German experts in wound healing let to their further development and improvement.The article reviews the need for education in the treatment of chronic wounds for medical personnel—nurses and doctors.Experiences and perspectives are discussed.

    • Study of intra-abdominal hypertension prevalence and awareness level among experienced ICU medical staff

      Abstract:Background:Intra-abdominal hypertension(IAH) is a disease with high morbidity and mortality among critically ill patients.The study’s objectives were to explore the prevalence of IAH and physicians’ awareness of the 2013 World Society of Abdominal Compartment Syndrome(WSACS) guidelines in Chinese intensive care units(ICUs).Methods:A cross-sectional study of four ICUs in Southwestern China was conducted from June 17 to August 2,2014.Adult patients admitted to the ICU for more than 24 h,with bladder catheter but without obvious intravesical pressure(IVP) measurement contraindications,were recruited.Intensivists with more than 5 years of ICU working experience were also recruited.Epidemiological information,potential IAH risk factors,IVP measurements and questionnaire results were recorded.Results:Forty-one patients were selected.Fifteen(36.59%) had IVP?12mm Hg.SOFA(Sequential Organ Failure Assessment) hepatic and neurological sub-scores were utilized as independent predictors for IAH via logistic backward analysis.Thirty-seven intensivists participated in the survey(response rate:80.43%).The average score of each center was less than 35 points.All physicians believed the IAH prevalence in their departments was no more than 20.00%.A significant negative correlation was observed between the intensivists’ awareness of the 2013 WSACS guidelines and the IAH prevalence in each center(r=-0.975,P=0.025).Conclusion:The prevalence and independent predictors of IAH among the surveyed population are similar to the reports in the literature.Intensivists generally have a low awareness of the 2013 WSACS guidelines.A systematic guideline training program is vital for improving the efficiency of the diagnosis and treatment of IAH.

    • A bibliometric analysis of exertional heat stroke research in Web of Science

      Abstract:Background:Exertional heat stroke is a fatal condition and remains a health problem.This paper evaluates the publication trend regarding exertional heat stroke research between 1996 and 2015 using a bibliometric method.Methods:Articles regarding exertional heat stroke research published between 1996 and December 2015 were searched for in the SCI-EXPANDED database of Web of Science.The search results were analyzed with regard to publication year;publication quantity regarding countries/regions;authors;citation frequency;and journal distribution.Cite Space(v3.6) was used for a document co-citation visualization analysis.Results:In total,289 publications on heat stroke were located.After selection,209 original articles conducted across 28 countries/regions and published in 83 journals were included in the analysis.The USA,Isreal,and France were the most common locations for exertional heat stroke studies.The Cite Space visualization cluster analysis showed that exertional heat stroke-related mortality and protective measures were constant concerns of research.Conclusion:Research related to exertional heat stroke has been continuous concerned.USA is still the leading country in this field.

    • Effect of westernization on oral health among college students of Udaipur City,India

      Abstract:Background:There is overwhelming evidence that periodontal disease and dental caries affect the majority of populations and that western culture and lifestyle may have a profound influence on oral health,especially in adults.The present study was performed to determine the effect of westernization on the oral health of college students of Udaipur City,Rajasthan.Methods:A descriptive cross-sectional study was conducted among students attending various professional and non-professional bachelor’s degree colleges of Udaipur City,Rajasthan,India,from March 2013 to May 2013.Eight hundred students were selected based on a two-stage random sampling procedure.Westernization was assessed by a self-administered structured questionnaire.Periodontal status,dental caries status and malocclusion were assessed according to World Health Organization(WHO) criteria(1997).Statistical analysis was performed using Chi-square and Multivariate logistic regression.The confidence level and level of significance were set at 95% and 5%,respectively.Results:The present study suggested that adverse habits,listening to English music and preferring English food had a significant association with dental caries and periodontal diseases.Malocclusion also showed a significant relationship with consuming English food for snacks and desserts.Multivariate analysis revealed a significantly greater odds ratio(OR) for periodontal disease and dental caries among those who preferred English food for lunch.Conclusion:Based on the results of the present study,there is an association between westernization and oral health.

    • Pathophysiology of sepsis-induced myocardial dysfunction

      Abstract:Sepsis-induced myocardial dysfunction is a common complication in septic patients and is associated with increased mortality.In the clinical setting,it was once believed that myocardial dysfunction was not a major pathological process in the septic patients,at least in part,due to the unavailability of suitable clinical markers to assess intrinsic myocardial function during sepsis.Although sepsis-induced myocardial dysfunction has been studied in clinical and basic research for more than 30 years,its pathophysiology is not completely understood,and no specific therapies for this disorder exist.The purpose of this review is to summarize our current knowledge of sepsis-induced myocardial dysfunction with a special focus on pathogenesis and clinical characteristics.

      Abstract:With key roles in essential brain functions ranging from the long-term potentiation(LTP) to synaptic plasticity,the N-methyl-D-aspartic acid receptor(NMDAR) can be considered as one of the fundamental glutamate receptors in the central nervous system.The role of NMDA R was first identified in synaptic plasticity and has been extensively studied.Some molecules,such as Ca2+,postsynaptic density 95(PSD-95),calcium/calmodulin-dependent protein kinase II(Ca MK II),protein kinase A(PKA),mitogen-activated protein kinase(MAPK) and cyclic adenosine monophosphate(c AMP) responsive element binding protein(CREB),are of special importance in learning and memory.This review mainly focused on the new research of key molecules connected with learning and memory,which played important roles in the NMDAR signaling pathway.

    • The origin and role of innate lymphoid cells in the lung

      Abstract:Innate lymphoid cells(ILCs),a newly identified member of the lymphoid population,play a critical role in the transition from innate to adaptive immunity in host defense.ILCs are important in mucosal barrier immunity,tissue homeostasis,and immune regulation throughout the body.Significant alterations in ILC responses in lung diseases have been observed and reported.Emerging evidence has shown that ILCs are importantly involved in the pathogenesis and development of a variety of lung diseases,i.e.,helminth infections,allergic airway inflammation,and airway hyper-responsiveness.However,as a tissue-resident cell population,the role of ILCs in the lung remains poorly characterized.In this review,we discuss the role of ILCs in lung diseases,the mechanisms underlying the ILCmediated regulation of immunity,and the therapeutic potential of modulating ILC responses.

    • Recovery from rabies,a universally fatal disease

      Abstract:Background:Rabies is a zoonosis transmitted via the bites of various mammals,primarily dogs and bats.Known since antiquity,this disease may have the deadliest human fatality rates and is responsible for approximately 65,000 deaths worldwide per year.Case presentation:We report the case details of a 13-year-old boy from India belonging to a South Asian ethnicity,who presented with altered sensorium one month following a dog bite.He did not receive the active rabies immunization and was managed with supportive therapy.The patient had extensive T2 weighted(T2W)/fluid attenuation and inversion recovery(FLAIR) hyper intensities involving the deep gray matter of the cerebral hemispheres,hippocampus,brainstem,and cerebellum.The diagnosis was confirmed by the demonstration of the rabies antigen from a nuchal skin biopsy and a corneal smear.The patient had a slow but significant recovery over four months and was discharged from the hospital in stable condition with severe neurological sequelae.Conclusion:We report a unique case of survival after infection with a universally fatal disease.

      Abstract:Background:Intussusception refers to a condition in which a segment of the intestine invaginates into the lumen of an adjacent segment of the intestine.Postoperative intussusception after gastrointestinal surgery is an uncommon clinical condition,and there is only one case report of small bowel intussusception after rectal cancer surgery.Here,we report a case of spontaneous small bowel intussusception following laparoscopic total mesorectal excision for rectal cancer.Case presentation:A 56-year-old female military officer was referred to the Colorectal Surgical Department for midrectal cancer,8cm from the anal verge.The patient underwent laparoscopic low anterior resection and diverting loop ileostomy.On postoperative day 3,the patient complained of vomiting and abdominal pain,and a follow-up abdomino-pelvic computed tomography scan showed an ileo-ileal type intussusception.After two days of surgical observation,the clinical symptoms were not resolved.The patient underwent exploratory laparotomy.On exploration,intussusception was found 40 cm proximal to the loop ileostomy site.Segmental resection of the ileum was carried out,and there was no pathological leading point on the resected ileum.The patient was discharged on postoperative day 14 after the second operation and has remained in good health for two years.Conclusion:We present a case of spontaneous small bowel intussusception after laparoscopic total mesorectal excision for rectal cancer that was treated by surgical resection 5 days after the index surgery.
