    • Lentiviral-mediated siRNA against osteopotin in U251 glioma cell line

      Abstract:Background:It has been reported that the expression of SPP1, the mRNA for osteopontin (OPN), is increased in a number of transformed cell lines and some tumors. Increased serum and plasma OPN levels were associated with advanced-stage carcinomas of the lung, breast, colon and prostate. The extent of OPN expression may be correlated with the malignancy degree of gliomas, but the specific role of OPN in the malignancy of gliomas remains to be clarified. Methods:In this study, we used a lentiviral system, which could induce a long-lasting down-regulation of gene expression, to knock down the OPN gene in human glioma cell line U251. And then we observed the proliferation and apoptosis of glioma cells. Results:OPN mRNA and protein were successfully knocked down in U251 cells, and the expression of caspase-3 and caspase-9 were increased about by 2 folds in OPN down-regulated U251 cells. 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium (MTT) assay showed that OPN down-regulation did not affect the proliferation of U251 cells. Conclusion:Our findings indicate that OPN might play a role in apoptosis, but not in cell proliferation in U251 glioma cells.

    • Effect of aspirin alone or combined with cisplatin on human cervical carcinoma HeLa cells

      Abstract:Objective: To study the effect and mechanism of aspirin alone or combined with cisplatin(DDP)on human cervical carcinoma HeLa cells. Methods: HeLa cells were treated by different concentrations of aspirin, DDP alone or both. The inhibitory effect on cell growth was analyzed by MTT and colony-forming assay. Cell apoptosis was measured with flow cytometry. The mRNA levels of Bcl-2, Bax and NF-κB(P65) were studied by RT-PCR. Results: MTT assay showed that aspirin inhibited HeLa cell proliferation in a time- and dose-dependant manner. Aspirin decreased clone numbers in colony formation assay. Aspirin also induced apoptosis of HeLa cells in a dose- and time-dependent manner as detected by flow cytometry. The inhibition effects on proliferation, colony formation and apoptosis were significantly enhanced when cells were treated with both aspirin and DDP. RT-PCR demonstrated that aspirin decreased the transcription of Bcl-2 and NF-κB, and increased expression of Bax gene. Conclusion: Aspirin can induce apoptosis in HeLa cells. Combination of aspirin and DDP displays a synergistic effect. The possible mechanism might be that aspirin downregulates the mRNA levels of Bcl-2 and NF-κB gene and upregulates the expression of Bax.

    • A model of Korotkoff sounds method electronic sphygmomanometer equivalent to the mercury one

      Abstract:Mercury sphygmomanometer (MSM) is reliable and widely used in clinics and hospitals. The principle of Korotkoff sounds method (KSM) applied in the MSM is also a gold standard to measure blood pressure. Many efforts have been made attempting to replace MSM, which is criticized for being not healthy and safe. In this research, an electronic blood pressure monitor, named K-sounds electronic sphygmomanometer (KESM), was designed as a substitute to MSM. The three key elements of KSM were proposed for the first time. We used appropriate electronic components to build the KESM which can fulfill the functions related to the three key elements. The KESM, which was easy to operate and free of mercury, followed the same principle as MSM. The same principle guaranteed the comparable accuracy. We took equivalence test and the results showed that the designed KESM was as accurate as the calibrated standard MSM. The designed KESM passed the certifications of SFDA and is qualified in clinics or hospitals for diagnostic purposes.

    • Anticoagulation early after mechanical heart valve replacement

      Abstract:Objective: To explore the changes of coagulation activity and the characters of anticoagulation early after mechanical heart valve replacement. Methods: All patients only took warfarin orally for anticoagulation. The predicted international normalized ratio (INR) was 1.5 to 2.0. Several coagulation markers were monitored early after valve replacement. Complications associated with anticoagulation were recorded and analyzed. The patients were divided into three groups based on the number and position of mechanical valve prothesis, including group M (mitral valve replacement), group A (aortic valve replacement) and group D (mitral and aortic valve replacement).Comparison was made between the three groups. Results: Three events of mild cerebral embolism and five events of mild bleeding occurred during the early postoperative period. One patient suffered from mild cerebral embolism on the 4th day after operation, accompanied by large volume of pericardial drainage. Two patients with bleeding had lower INRs than predicted range. However, INR in one patient with mild cerebral embolism was in the predicted range. There was no significant difference in thrombo time (TT), activated partial thromboplastin time (APTT) and INR on the 3rd day after operation compared to those before operation; meanwhile, plasma fibrinogen (FIB) concentration was higher than that before operation (P<0.05). INR had no significant changes on the 2nd day after the beginning of anticoagulation compared to that before operation; however, INR was significantly elevated on the 4th day (P<0.05). Warfarin doses and INRs were similar among the three groups, but FIB concentrations in plasma were higher in groups M and D than in group A (P<0.01). Conclusion: Hypercoagulabale state exists early after mechanical heart valve replacement. When anticoagulation begins is determined by the change of coagulation markers, not by the volume of chest or pericardial drainage. INR can not accurately reflect the coagulation state sometimes, especially during the first 3 days after anticoagulation. The number and position of mechanical valve prothesis could affect coagulation state. Therefore, anticoagulation therapy should be regulated accordingly.

      Abstract:Objective: The aim of the present study was to assess the early clinical outcome and risk factors in old patients with acute ST elevation myocardial infarction (STEMI) following primary percutaneous coronary intervention (PCI). Methods: A total of 136 patients older than 60 years with STEMI who received successful PCI were included in this study. The patients were classified in 2 age groups: patients ≥75 years and <75 years of age. The extent of coronary artery lesions was measured by quantitative coronary artery angiography (QCA). Subjects were tracked for subsequent cardiovascular events: cardiac death, myocardial infarction, heart failure, percutaneous coronary intervention, coronary artery bypass and stroke. Results: Though the older group had a higher prevalence of adverse baseline characteristics and lower final TIMI flow than patients<75y (P<0.05), the procedural success did not make difference between the two groups. In 12 months follow-up of 136 study participants, there occurred 39 CV events : cardiac death (five patients), heart failure (nineteen patients), and stroke (six patients). Three patients received coronary bypass grafts and six patients underwent PCI. Heart failure and overall cardiovascular event rates were higher in older patients compared with those in patients<75y. The main adverse clinical events (MACE) for the old group were a little higher comparing with the younger in 12-month follow-up (P=0.029 6 and P=0.043 4). Multivariate cox analysis identified that a diagnosis of diabetes (HR 2.495, 95%CI 1.224 to 5.083, P= 0.011 8) and time from symptom(HR 1.450, 95%CI 1.143 to 1.841, P= 0.008 2) to PCI as independent predictors of CV events after adjustment of all entered baseline variables. Conclusion: Our study suggests that drug-eluting stent implantation in older patients with acute ST elevation myocardial infarction has high initial procedural success rates despite having more severe baseline risk characteristics, and to shorten the time from symptom onset to PCI may decrease cardiovascular events in old patients following PCI.

      Abstract:Objective:To study the influences of different time intervals between loop electrosurgical excision (LEEP) and abdominal hysterectomy or radical hysterectomy on postoperative complications. Methods:Sixty-eight patients, who received subsequent abdominal hysterectomy or radical hysterectomy after LEEP due to CIN Ⅲ and cervical cancer (IA1, IA2 and IB1), were included in the present study. The hospital and clinic records of these patients were reviewed. The patients were divided into three groups according to the time intervals between LEEP and hysterectomy or radical hysterectomy: group 1(within 48 h), group 2 (between 48 h to 6 weeks), and group 3(> 6 weeks ).Results: General characteristics of patients, including the mean age, delivery history, BMI, menopausal status, clinical stage and HPV infection, were comparable between patients of different groups. There were no significant differences in the mean transfusion amount, posthysterectomy hospital stay or operation time between different groups. The frequencies and spectrum of complications were not significantly affected by the time interval between LEEP and hysterectomy or radical hysterectomy. Conclusion: It is concluded that whenever the LEEP is done, the operation including hysterectomy or radical hysterectomy can be conducted at any time as it is necessary for the patients.

    • Undergraduate nursing education in the United States

      Abstract:Nursing education in the United States began in the 1870s with hospital training schools modeled after the Nightingale plan. Presently there are three educational paths that students may take to become a registered nurse (RN). These are the baccalaureate degree (BS), the associate degree (AD) and the diploma. Accelerated nursing programs are also available for people who have a baccalaureate degree in another field. Nursing education and nursing practice in the United States is regulated by each of the 50 states. The focus of nursing education is the preparation of nurses who use critical thinking skills to provide comprehensive patient-centered care to a variety of clients. It is expected that nurses will function using evidence-based practice in a culturally competent, safe manner. Nursing schools must be approved by the state in which they exist. In addition to this, most nursing schools are also accredited, a voluntary process that is done by a non-governmental association. All graduates take the same licensing exam, a computer adaptive test that is from 75 to 265 items long. In 2008 almost 120,000 US educated graduates were first time test takers; 85.5% passed the exam. Trends in nursing education include a focus on evidence based practice, an emphasis on safety and quality, patient education and management of chronic conditions, genetics and genomics and the use of simulation in nursing education.

      Abstract:A 47-year-old woman, who had undergone cardiac myxoma removal surgery six years ago, was readmitted due to a 2-week history of chest distress, and dyspnea with the change of body position. Both the examinations and the surgery confirmed a cardiac tumor relapse. Histology demonstrated that the mass resected in surgery had a pathologic change from myxoma to a low-grade malignant myofibroblastic sarcoma. The patient received heart transplantation 2 years later due to tumor recurrence, and finally died of cerebral hemorrhage.

      Abstract:Here we report a successful protocol in treatment of a patient with primary plasma cell leukemia (PPCL) using haploidentical stem cell transplantation (hi-HSCT). During first complete remission after routine chemotherapy, the patient received autologous blood stem cell transplantation, but he had relapse later. He gained a second CR after chemotherapy and underwent hi-HSCT from his daughter, who had HLA mismatched at three loci. Recovery of hemopoiesis was found at day 14 and complete donor chimerism was confirmed by PCR-STR on day 34, 95 and 238. The patients have survived disease-free for 56 months since hi-HSCT, without serious graft-versus-host-disease.

      Abstract:We report a case of Chylous ascites after whole pelvis suspension and to discuss the possible reasons. An 85-year-old female patient with anterior vaginal prolapse Ⅳ (AVP Ⅳ), vaginal vault proplapse Ⅳ and posterior vaginal prolapse Ⅳ (PVP Ⅳ), developed Chylous ascites 2 weeks after she received whole pelvis suspension. The cause of chylous ascites in the present case was not associated with malignant tumor, hepatic cirrhosis, or inflammation. Therefore, surgical trauma may be considered the cause of disorder. X-ray lymphangioadenography should be performed to draw a final conclusion.
