    • Prediction of HLA-A 2.1-restricted CTL epitopes from IGFBP7 antigen of lung carcinoma

      Abstract:Objective:With the development of peptide-based cancer specific immunotherapy,the prediction of CTL epitopes from insulin-like growth factor-binding protein 7(IGFBP7) is very important for some research about tumor metastasis.Because HLA-A2.1-expressing individuals cover >50% in the population of China,we aimed at identifying IGFBP7-encoded peptide presented by HLA-A2.1.Methods:In our study,a HLA-A2.1 restricted CTL epitope was identified by using the following two-step procedure:(a) computer-based epitope prediction from the amino acid sequence of IGFBP7 antigen;(b) Validation with epitope molecular modeling.Results:We obtained four epitopes with high immunogenicity scores by all of the three algorithms,i.e.,BIMAS,SYFPEITHI and IMTECH.Each of the four candidates satisfied the criteria of the HLA-A2.1-restricted CTL epitopes in molecular modeling analysis.Conclusion:The combination of BIMAS,SYFPEITHI and IMTECH method can improve the prediction efficiency and accuracy.Due to this research herein,this four epitopes have potential value for further studied,also have potential application in peptide-mediated immunotherapy.These epitopes may be useful in the design of therapeutic peptide vaccine for lung carcinoma and as immunotherapeutic strategies against lung carcinoma after identified by immunology experiment.

    • TLR4 -2242 T→C variant increases transcriptional activity of its promoter

      Abstract:Objective:To investigate the effects of -2242,-1892 and -1837 single nucleotide polymorphisms(SNPs) on toll-like receptor 4(TLR4) promoter activity.Methods:Polymerase chain reaction(PCR) and site direct mutation technology were used to construct TLR4 basic promoter and -2242C,-1892A and -1837G mutate promoter plasmids.Dual-Luciferase Reporter assay system was used to detect the activity of constructed promoter following human embryonic kidney(HEK) 293 cells were transiently cotransfected with the constructed plasmids and the control plasmid pRL-CMV.Results:In HEK293 cells,the activity of -2242C mutate promoter was higher than -2242T promoter,and there was no significant difference when both -1892A and -1837G mutate promoter compared with -1892G and -1837A promoter,respectively.Conclusion:It is implied that -2242T→C base variation can enhance the activity of TLR4 promoter,while -1892 and -1837 SNPs have no effect on TLR4 promoter activity.

    • Expression,purification and identification of LBD domain of human PPARδ in E.coli

      Abstract:Peroxisome proliferator-activated receptors(PPARs) are ligand-activated transcription factors known to play a pivotal role in regulations of metabolism.In order to yield soluble ligand binding domain of PPARδ(PPARδLBD) for screening ligands,the cDNA was amplified using total RNA from HepG2 cells by RT-PCR.Then the enzyme-digested product was inserted downstream of the malE gene in the vector pMAL-p2x,which encoded maltose-binding protein(MBP),resulting in the expression of an MBP-PPARδLBD fusion protein.The recombinant plasmid was transformed into E.coli TB1 that was cultured shakily at 30 °C,200 r/min and induced by 0.4 mmol/L IPTG for 6 h.The cells were harvested by centrifugation and broken by sonication.The expressed fusion protein was soluble and accounted for 0.31 of the total protein in the supernatant.Western blot analysis showed that the expressed MBP-PPARδLBD could bind to anti-MBP-antibody.The MBP-PPARδLBD fusion protein of 77 kDa and the PPARδLBD protein of 34 kDa were obtained by amylose-resin affinity chromatography without or with digestion of Factor Xa.They were both homogeneity,judged by SDS-PAGE.The recombinant MBP-PPARδLBD and PPARδLBD protein with high purity is obtained,which provides the necessary material for screening and researching PPARδ ligands.

    • Ex vivo differentiation of multipotent adult progenitor cells to skin epidermal cells

      Abstract:Objective:By establishing the indirect contact co-culture system,we studied the in vitro condition for MAPCs differentiating into epidermal cells and the transformation of MAPCs into epidermal cell phenotype.Methods:Cell culture insert membrane was used for substitute basal membrane and MAPCs,fibroblast cells(FCs) and mixture of MAPCs and epidermal cells and FCs were separately implanted into 2 sides of it.PKH26 was used to label cloned MAPCs;type IV collagen rapid adhering method was used to isolate and culture the skin epidermal cells from 1-day-old SD rat.Results:Part of the MAPCs transformed into cells expressing keratin in the presence of peripheral epithelia and FCs.Type IV collagen rapid adhering method successfully selected rats’ epidermal stem cells.The mixture of the 2 kinds of cells or indirect culture might promote the differentiation through mesenchymal factors secreted by dermis FC.Conclusion:We were the first to have established the in vitro model of MAPCs differentiation into epidermal cells,in which MAPCs were transformed into epithelium-like cells.

    • Analysis on blood glucose excursions in well-controlled patients with type 2 diabetes mellitus

      Abstract:Objective:To probe glycemic excursions in type 2 diabetic patients whose hemoglobin A1c(HbA1c) was kept ≤6.0%.Methods:Totally 36 cases with type 2 diabetes(T2DM) with HbA1c≤6.0% and 30 cases with normal glucose tolerance(NGT) from December 2005 to December 2007 in our department were subjected prospectively.Continuous glucose monitoring system(CGMS) was employed to record their continuous blood glucose level for 3 d.The blood glucose profiles including the mean blood glucose(MBG),standard differentiation(SD),mean amplitude of glycemic excursions(MAGE) and absolute means of daily differences(MODD) were analyzed.Results:T2DM group had obviously postprandial hyperglycemia,in about 2 h after meal,especially after breakfast.Fifty-two hypoglycemic episodes occurred during the monitoring period in T2DM groups,of which 73.1%(38 episodes) were absence of symptomatic hypoglycemia with the lowest value of blood glucose only 2.0 mmol/L.And 20 episodes took place during the day hours,while 32 episodes observed during the night hours.Compared with NGT groups,SD,MAGE and MODD were all significantly higher in T2DM groups.MBG was significantly correlated with HbA1c in T2DM groups,but SD,MAGE,NGE and MODD were all independent of HbA1c.MAGE was independent of MODD.Conclusion:The amplitude of glycemic excursions is higher in normol-controlled T2DM groups than the NGT groups.Thus ideally glycemic control is not only to make HbA1c reach standard,but also to lessen glycemic excursions and reduce hypoglycemia episodes.

    • Surgical techniques for improving hemostasis during Bentall procedure

      Abstract:Objective:To evaluate the effect of modified surgical techniques on hemostasis used in aortic root replacement with a composite graft(Bentall procedure).Methods:Data on 15 patients who underwent Bentall procedure during 2005 to 2007 were analyzed.The first 5 patients(Group 1) received the standard procedure.Then next 10 patients(Group 2) received the modified procedure.Techniques including "tandem suture line","endo-button buttress","sandwich anastomosis" and "left ventricle filling" were added to the standard procedure.Perioperative bleeding and the volume of blood transfusion required were compared to estimate hemostasis in different groups.Results:Between groups 1 and 2,a significant difference was found in postoperative bleeding [(2193±383) ml vs(1012±258) ml,respectively;P<0.05] and in volume of blood transfusion required [(7242±1416) ml vs(2520±708) ml,respectively;P<0.05].Conclusion:The modified surgical techniques used in our study are effective in the improvement of the hemostasis in Bentall procedure.

      Abstract:Objective:The Consolidated Standards for Reporting of Trials(CONSORT) statement has already proved to be an efficient standard for reporting quality of randomized controlled trials(RCTs).However,most of the Chinese medical journals have not endorsed the CONSORT statement.The current situation about the reporting quality of RCTs in Chinese medical journals is still unclear.The purpose of the study was to evaluate the reporting quality of RCTs on papers published in 5 leading Chinese medical journals.Methods:We evaluated 232 original RCT papers using a reporting quality scale based on CONSORT statement from 2001 to 2006 in 5 Chinese medical journals(Journal type 1) without adoption of CONSORT and Chinese Journal of Evidence-based Medicine(Journal type 2) which adopted CONSORT in 2004.We measured the inclusion of 26 items for the reporting quality scale and 6 core items of each RCT report,gave score to each item and calculated the total score obtained in each report and the proportion of reports including individual items.The reporting quality of RCT trials from 2001 to 2003(pre-adoption period) was compared with that from 2004 to 2006(post-adoption period).Results:The average reporting quality of RCTs was moderate(mean score,15.18),and the mean score of the 6 core items was low(mean score,1.09) in 5 leading journals.The difference in the total score and the score of the 6 core items between pre-adoption period(2001-2003) and post-adoption period(2004-2006) was statistically significant(P=0.003;P=0.000).Interaction between journal type and period was not significant(F=0.76;P=0.383).We concluded that the change tendency of reporting quality between Journal type 1 and 2 was not different.But as to the core items of sequence concealment and intention-to-treat analysis,the increases were greater for Journal type 2 when evaluated against Journal type 1(P=0.038;P=0.016).Conclusion:The reporting quality of RCT trials in 5 leading Chinese medical journals is improving.However,the lack of important items in RCT trials remains a serious problem.We recommend the endorsement of the Consolidated Standards for Reporting of Trials statement in Chinese medical journals and the continuing education on evidence-based medicine in China.

      Abstract:Cardiac surgery is a very common operation nowadays all over the world.Median sternotomy is a routine procedure required for cardiac access during open heart surgery.The complications of this procedure after the cardiac surgery range from 0.7% to 1.5% of all cases,and bear a high mortality rate if they occur.Every individual surgeon must pay great attention on every detail during the sternal closure.This article shows the details as to conventional information and updated progress on median sternotomy closure.The update contents involve in biomechanics,number of wires twists,biomaterial and so on.According to our experience,we recommend four peristernal single/double steel wires for sternal closure as our optimal choice.

      Abstract:Toll-like receptors(TLRs) are a central component of innate immune system and play a major role as the initiator of the innate immune responses to defend against bacteria,viruses,parasite and other pathogens.During malaria infection,TLRs signaling pathways are initialed with the recognition of Plasmodium glycosylphosphatidylinositols(GPI) and hemozoin as pathogen-associated molecular patterns(PAMPs).And then,activation of TLRs signaling induces specific biological responses against malaria parasites invasion.However,TLRs are also involved in malaria pathogenesis and enhancement of immune tolerance and evasion for malaria infection.Moreover,malaria parasites regulate selectively TLRs expression on immune cells.Thus,these evidences indicated that TLRs have contrary roles on malaria infection.Understanding the complicated roles of TLRs on malaria infection will contribute us to design more effective anti-malaria drugs or vaccines.
